SUPPLY AGREEMENT FOR LIQUID MEDICAL OXYGEN
RFP NUMBER: FHI/EPIC/2024/001

A. BACKGROUND
FHI 360 is a non-profit human development organization dedicated to improving lives in lasting ways by advancing integrated, locally driven solutions. Our staff includes experts in health, education, nutrition, environment, economic development, civil society, gender, youth, research, and technology; creating a unique mix of capabilities to address today’s interrelated development challenges. FHI 360 serves more than 60 countries, all 50 U.S. states and all U.S. territories.

The Meeting Targets and Maintaining Epidemic Control (EpIC) is a five-year, global project funded by the U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) and the United States Agency for International Development (USAID). EpiC Nigeria’s goal is to deliver efficient, affordable, results-based technical assistance and direct services while closing remaining gaps in Nigeria’s efforts to meet HIV epidemic control targets and address the COVID-19 pandemic.

Part of EpIC COVID-19 program in Nigeria is to provide high quality, medical-grade oxygen supply to ten (10) supported facilities in Nigeria namely, 1. Federal Medical Centre Owerri Imo State, 2. University of Uyo Teaching Hospital, Uyo Akwa Ibom State, 3. Federal Medical Centre Abeokuta Ogun State, 4. Federal Medical Centre Yenagoa Bayelsa State, 5. Abubakar Tafawa Balewa University Teaching Hospital, Bauchi State, 6. Federal Medical Centre, Asaba Delta, 7. National Hospital Abuja, FCT, 8. Dalhatu Araf Specialist Hospital Lafia -Nasarawa State State, Jos University Teaching Hospital Jos – Plateau State, 10. University Teaching Hospital Port Harcourt-Rivers State. FHI 360 is seeking to establish Liquid Medical Oxygen Supply Agreement for six (6) of 8,000 liters) to each storage facility or 48,000 liters in total per facility over a period of 6 months. The transported LOX will be delivered to the existing Cryogenic Tank in the supported facilities Experienced and Interested suppliers/producers of medical-grade liquid oxygen in Nigeria are required to follow the below guidance to participate in the bidding.

B. QUALITY AND SPECIFICATION FOR LIQUID OXYGEN

C. PROPOSAL REQUIREMENTS AND SELECTION CRITERIA
Each proposal to be submitted must include the below criteria on your company letter head in the following order;

Legal Status
• Documents showing that supplier/producer is legally registered and in good standing which include copy of Corporate Affairs Commission form CO2 and C07 and Tax Certificate for year 2020, 2021 and 2022.
• Documents is sued by the relevant government entity (les) showing that the supplier/producer is authorized to supply or produce and store medical oxygen in Nigeria.

Technical proposal
• Clearly describe in your company’s letterhead the ability to perform the scope of services proposed, that is, Description of how medical oxygen is stored and distributed from production through final delivery.
• Bidders (who are not manufacturer of LOX) are expected to provide information regarding the source of supply of oxygen as mentioned in table 2 Including experience, competency, and installed capacity for production of medical oxygen production In Nigeria.
• Bidder are expected to develop 6 months’ supply plan for medical oxygen to all the 10 sites.
• Submit ISO Certificate and documentation verifying that oxygen tanks (storage and distribution) are manufactured to appropriate standards, inspected regularly, and is labeled accordingly.

Delivery Turnaround time
• Please indicate the turnaround time for each supply to each of the facility and develop a supply plan of six (6) deliveries to each of the support site stated above.

Financial Proposal – Cost
Proposers/bidders shall submit a competitive quote based on the six (6) deliveries of 8,000 liters’ supply plan for each facility.
Please note that FHI 360 is VAT exempted but will deduct withholding tax from all payments in compliance with the tax laws.

Validity of Quote – State how long your quote is valid for.

Payment Terms – State the payment terms.

D. INSTRUCTION TO BIDDER
• All proposals (technical and financial) must be submitted on company letterhead signed by the authorized representation of your company and in PDF format as one file/document.
• Submission of all documents required shall be made to [email protected] not later than 5.30pm May 7th, 2024.
• The tile of your email submission should be “Proposal for Supply of Liquid Medical Oxygen – FHI/ EPIC/2024/001”
• Enquiry on this Invitation to Bid shall be by email communication to [email protected] on or before 5.30pm May 1st, 2024.

E. PLEASE NOTE
• Issuance of this RFP in no way constitutes a commitment by FHI 360 to any bidder.
• FHI 360 reserves the right to vary any part of this RFP at the time of award if it becomes necessary.
