REQUEST FOR QUOTATION FOR DATA COLLECTION FIRM FOR IPC OBSERVATION DATA FOR WASH PROGRAM
BACKGROUND
Jhpiego Corporation, an affiliate of John Hopkins University, is an international non-profit non-governmental organization that works to improve the health of women and families throughout the world.
Jhpiego Corporation is seeking for data processing firm to provide services of data collection for the Jhpiego MCGL Wash program. Vendors are encouraged to read the entire document to ensure that they have a clear understanding of Jhpiego requirements before submitting a bid.

2.1 QUOTATION SHOULD INCLUDE THE FOLLOWING FOR REQUIRED SERVICES:
• The quotation should be enclosed in a plain sealed envelope, with the item being quoted for written boldly on the right-hand side of the envelope.
• Proof or license to offer required services as given by the responsible regulatory body (if applicable).
• Lead-time for Supply
• Quote validity-minimum 12 months
• Payment terms if different (preferably Net 30 days).
3.1 CONTRACT DURATION
• At the successful completion of the data collection process.
4.1 ELIGIBILITY
All applicants should provide the following;
• Certificate of registration/incorporation, memorandum and articles of association and valid Tax Identification Number (TIN).
• Vendor must be a licensed/authorized vendor for services to be rendered (please provide supporting documentation),
• Company profile (briefly reflecting the core competencies, key management personnel and clearly stating the physical verifiable office address, mobile number and e-mail address).
• Proof or license to offer required services as given by the appropriate regulatory body (if applicable).
• Minimum of three (3) reputable references active within the last three years form clients/organizations
• Note that applicable government taxes will be deducted prior to making payments for items supplied.

Ensure that you put alt quotes into bid box at the reception. Also, ensure that you complete the bid work sheet at the reception.
6.1 COMMUNICATION:
Interested firms should kindly send an e-mail to [email protected] requesting for the TOR and further guidance on details of submission. E-mail subject should be “REQUEST FOR DATA COLLECTION FIRM TOR-WASH”.
In addition, we will take questions via email above only up to 4 July, 2022. All candidates will receive responses/answers on or before 5 July 2022, while final Proposal submissions in hard copies only are due by 4pm 7 July, 2022.
7.1 CLOSING DATE
The Closing date for the submission of bids is at 4pm WAT on 7 July, 2022.
