REQUEST FOR QUOTATION FOR ANNUAL PREVENTIVE MAINTENANCE AND CERTIFICATION OF PCR LABORATORY EQUIPMENT
BACKGROUND.
Jhpiego Corporation, an affiliate of John Hopkins University, is an international non-profit, non-governmental organization that works to improve the health of women and families throughout the world.
Jhpiego Corporation is seeking for reputable vendors for provision of annual preventive maintenance and certification of PCR laboratory equipment in Taraba State. Vendors are encouraged to read the entire document to ensure that they have a clear understanding of Jhpiego requirements before submitting a bid.
1.1 ITEM DESCRIPTION

2.1 QUOTATION SHOULD INCLUDE THE FOLLOWING:
• The quotation should be enclosed in a plain sealed envelope, with the item being quoted for written boldly on the right-hand side of the envelope.
• Proof or license to offer required services as given by the responsible regulatory body (if applicable),
• lead-time for Supply
• Quote validity of at least 60 days.
• Payment terms if different (preferably Net 30 days)
3.1 CONTRACT DURATION
• At the successful delivery of all items as on the approved Purchase Order.
4.1 ELIGIBILITY
All applicants should provide the following;
• Certificate of registration/incorporation, memorandum and articles of association and valid Tax Identification Number (TIN).
• Vendor must be a licensed/authorised vendor for services to be rendered (please provide supporting documentation)
• Company profile (briefly reflecting the core competencies, key management personnel and
• clearly stating the physical verifiable office address, mobile number and e-mail address).
• Proof or license to offer required services as given by the appropriate regulatory body (if applicable).
• Minimum of three (3) reputable references active within the last three years form clients/organizations
• Note that applicable government taxes will be deducted prior to making payments for items supplied.

Ensure that you put all quotes into bid box at the reception. Also, ensure that you complete the bid worksheet at the reception.
6.1 COMMUNICATION:
Interested firms should kindly send an e-mail to [email protected] requesting tor the Scope of Work (SOW) and further guidance and process for submission. E-mail subject should be “Certification of PCR laboratory equipment”.
In addition, we will take questions via email up to August 10, 2022 All candidates will receive responses/answers on August 15, 2022, while final proposal submissions are due by 4:00 pm August 18, 2022.
7.1 CLOSING DATE
The closing date for the submission of bids is at 4pm WAT on August 18 2022
